Common Causes of Shoulder Pain

Shoulder pain can originate from various sources, including rotator cuff injuries, frozen shoulder, arthritis, or even referred pain from neck issues.

  1. Rotator Cuff Tendinitis: This is a condition characterized by inflamed tendons in the shoulder, often due to overuse or repetitive motion.
  2. Frozen Shoulder: Also known as adhesive capsulitis, this condition involves stiffness and pain in your shoulder joint.
  3. Shoulder Hypermobility: This occurs when the ball and socket of the shoulder joint are looser than normal – this could lead to dislocation.
  4. Shoulder Dislocation: If your shoulder is pulled back too hard or rotated too far, the top of your arm might pop out of its socket.
  5. Shoulder Bursitis: Bursitis is inflammation of the bursa, a small, fluid-filled sac that reduces friction in the shoulder joint.
  6. Shoulder Arthritis: There are many types of arthritis, but osteoarthritis is the most common type affecting the shoulder, causing pain and stiffness.
  7. Fracture (Broken Bone): A fracture in the shoulder joint can cause severe pain.
  8. Calcific Tendonitis: This condition is characterized by calcium deposits forming in the tendons of the shoulder, causing pain.
  9. Shoulder Impingement: This occurs when the top of the shoulder blade puts pressure on the underlying soft tissues when the arm is lifted away from the body.

Symptoms of Shoulder Pain

Common symptoms of shoulder pain include discomfort and stiffness, difficulty raising the arm, weakness, and sometimes a clicking sound during movement.

  1. Pain: This can range from a dull ache to a sharp or severe pain. The pain may be constant or it might only occur when you move your shoulder.
  2. Stiffness: You might find it difficult to move your shoulder as freely as usual, or you may not be able to move it in its full range of motion.
  3. Weakness: You might find it harder to carry objects or use your arm as you normally would.
  4. Swelling and Inflammation: The shoulder area may appear swollen or feel warm to the touch.
  5. Decreased Mobility: Difficulty in performing everyday activities that involve moving the shoulder, such as reaching for something on a high shelf or combing your hair.
  6. Nighttime Discomfort: Many people with shoulder pain report worse symptoms at night, which can affect sleep quality.
  7. Clicking or Popping Sensation: Some people may hear a clicking or popping sound when they move their shoulder.

If you’re experiencing these symptoms persistently, it’s crucial to seek medical attention. Neglecting these symptoms can escalate the problem, leading to chronic issues and further complications.

How Do I Know If My Shoulder Pain is Serious?

While shoulder pain is common and often not a cause for serious concern, there are certain signs and symptoms that could indicate a more serious issue. It’s important to seek immediate medical attention if you experience any of the following:

  1. Severe Pain: If your shoulder pain is intense and sudden, or gets progressively worse, it could be a sign of a serious condition such as a fracture or a severe strain or sprain.
  2. Persistent Pain: Shoulder pain that doesn’t improve even after a few days of rest, icing, and over-the-counter pain relievers, could be a sign of a more serious issue.
  3. Inability to Use the Arm: If your shoulder pain is accompanied by difficulty moving the arm or carrying objects, this could indicate a serious injury.
  4. Deformity of the Joint: If your shoulder joint looks dislocated or visibly out of place, seek medical attention immediately.
  5. Signs of Infection: Symptoms such as redness, swelling, warmth in the area, or fever could suggest an infection.
  6. Numbness or Tingling: These sensations could indicate nerve damage or compression.
  7. Pain at Night or at Rest: Pain that disturbs your sleep or occurs when you’re not using your shoulder could be a sign of a serious condition.

Remember, while this guide can provide some indications, it’s always best to consult with a healthcare provider to get a proper diagnosis and treatment plan.

Physiotherapy Treatment Options for Shoulder Pain

Physiotherapy offers a comprehensive range of treatment options that can be tailored to your specific needs. These may include:

  • Manual therapy: Techniques such as joint mobilizations and manipulations can help restore movement. For instance, a physiotherapist may use gliding techniques to move the joint in specific directions to regain its normal range of motion.
  • Exercise therapy: Specific strengthening and flexibility exercises can improve your shoulder’s function. For example, resistance band exercises can strengthen the muscles around the shoulder, promoting stability and reducing pain.
  • Electrotherapy: Modalities like TENS (Transcutaneous Electrical Nerve Stimulation) or ultrasound can reduce pain and inflammation. TENS uses low voltage electric currents to relieve pain, while therapeutic ultrasound uses sound waves to promote tissue healing.

The Benefits of Physiotherapy for Shoulder Pain

Physiotherapy offers numerous benefits for individuals suffering from shoulder pain:

  • Reduced Inflammation: Physiotherapy techniques can help manage swelling and inflammation, two common culprits of shoulder pain. Manual lymphatic drainage, for example, can help reduce swelling by encouraging the natural drainage of lymph fluid.
  • Increased Range of Motion: Regular physiotherapy sessions can significantly improve your shoulder’s mobility. Stretching and strengthening exercises can enhance flexibility and prevent stiffness, helping you regain your normal activities.
  • Pain Relief: Physiotherapy can provide immediate and long-lasting relief from pain. Techniques such as soft tissue massage and heat therapy can soothe muscle tension and alleviate pain.

Home Exercises for Shoulder Pain

Alongside in-clinic treatments, home exercises play a vital role in managing shoulder pain. Here are some general exercises that our physiotherapists often recommend:

  1. Pendulum Exercise: Stand and lean forward slightly, allowing one arm to hang down and swing in a small circle. Perform 10 revolutions in each direction, once a day.
  2. Shoulder Rolls: Start by lifting your shoulders up towards your ears, then slowly roll them back while squeezing your shoulder blades together, and finally bring them down. Repeat this 10 times, twice a day.
  3. Wall Push-ups: Stand in front of a wall, place your hands flat against it, slightly wider than shoulder-width apart. Then, keeping your body straight, bend your elbows to bring your body towards the wall. Push back to the start position. Do this 10 times, twice a day.
  4. Doorway Stretch: Stand in an open doorway, reach your arms out to the sides and place your hands on the door frame. Keeping your arms in place, take a step forward until you feel a stretch in the front of your shoulders. Hold for 30 seconds, repeat twice a day.
  5. Resistance Band Pulls: Hold a resistance band in front of you with both hands shoulder-width apart. Keeping your arms straight, pull the band towards your chest by moving your arms outward. Slowly return to the starting position. Repeat this 10 times, twice a day.

Consistent practice of these exercises can help manage shoulder pain and prevent future occurrences. It is important to always perform these exercises within your comfort range and consult with your physiotherapist if any discomfort occurs.
